Make a Referral
Referrals can be made by commissioners, professionals, families or individuals. You can contact us by phone, email or through our online referral form. Once we receive your enquiry, a member of our team will be in touch to discuss your needs.
Initial Review
We will carry out an initial review of the information provided to understand the individual’s needs, preferences and circumstances. This helps us determine whether our services are the right fit and how we may be able to support.
Assessment
If appropriate, we will arrange a full, person-centred assessment. This may involve meeting the individual, their family and relevant professionals to gain a full understanding of support needs, risks, goals and aspirations.
Care & Support Planning
Working closely with everyone involved, we develop a tailored support plan focused on promoting independence, wellbeing and quality of life. The plan outlines how support will be delivered and how outcomes will be achieved.
Placement & Transition
Once all parties are in agreement, we carefully plan the transition into the service. This includes introductions, staff matching and a gradual move where needed to ensure the individual feels safe, comfortable and supported.
